05-MARCH-2015 FATHIYEH HASAN AL-JAWAD 69 YEARS
EXTRUDED DISC L4-5 WITH LEFT UPWARD MIGRATION AND SEVERE STENOSIS.
Anamnesis
The patient came to the clinic 31-January-2015
complaining of LBP with agonizing left sciatica
the last 2 weeks down to calf muscle. The
patient was operated by me
14-July-2014 for huge extruded disc C5-6
with malacia of the spinal cord more to the
right.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 50
degrees in the right and 5 degrees in the left
with pain. The left AJ is absent and there is
weak dorsiflexion left foot 3/5 and right foot
4/5.
The patient sent for investigations: MRI of the
lumbar spine done 01-February-2015 showing huge
extruded disc L4-5 with left upward migration
with segmental stenosis.
Decompressive laminectomy L4 and lower 4/5 of
L3. Foraminotomy L4 and L5 left side. The upward
extruded disc was removed and left sided
intradiscal cleaning L4-5 was achieved. Routine closure of the wound.
Smooth postoperative recovery. The power of both
feet became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
