13-JULY-2015 HIND MUHAMED SALAH 55 YEARS
EXTRUDED DISC C6-7 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient was operated by me
28-March-2006
for extruded disc C5-6 then for right CTS, then
left CTS 16-February-2015. MRI cervical spine
done 02-April-2015 showing small extruded disc
C4-5 and C5-6. There was no sensory motor
deficit of both upper limbs. The patient then
came 12-July-2015 complaining of agonizing neck
pain with inability to move the head to all
directions with shooting pain to the right upper
limb.
On examination: The patient is in agonizing pain
with weak grip and extension of the right hand
4/5 and right triceps 3/5. There is also weak
dorsiflexion right foot.
MRI of the cervical spine performed today,
showing extruded disc C6-7 with left foraminal
occlusion with small extruded disc C4-5, bony
hard to the left. The previously operated disc
of C5-6 disappeared as the patient ha
assimilation of C5-6.
The old incision disappeared, for what the
supposed level was was identified and discectomy
of C6-7 was performed with removal of the
extrusion at the right foramen. The dura was
seen at the entire posterior aspect of the disc
space. A compact anatomical cage 5 mm
inserted to the disc space and fusion of C6-7
was achieved using Atlantis one level plate 25
mm with 4 screws 4x15 mm. Routine closure
of the wound.
Smooth postoperative recovery. The patient was
sent to the floor.
Comments
The patient had small PCD C6-7 which was
silent 2 months ago. The last 3 days fresh extrusion of this
area to the right foramen was severely compressing the root,
which necessitate the surgery.
