EXTRUDED DISC L5-S1 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 13-February-2016
complaining of acute LBP with left sciatica with
numbness lateral toes left foot.
He was operated elsewhere for extruded disc
L4-5 1990 for left sciatica.
On examination: the patient is limping with
exaggerated scoliotic
stance. SLRS was 65 degrees with pain in the
right and 35 degrees with more pain in the left. There is weak dorsiflexion
left foot 4/5 and
planterflexion same foot 4/5 and hypalgesia
left S1 territory.
The patient was sent for MRI lumbar spine. It
was preformed the same day
showing extruded disc L5-S1 with left foraminal occlusion.
The old incision was 30 cm
length. Projectional incision over the old one.
The dissection was carried out over the scar,
until the lamina of L5 was identified. Left S1
foraminotomy and the huge extruded disc was
removed lateral to the axilla. Left sided
intradiscal cleaning L5-S1 disc space. Using MultiGen, a
bipolar pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 msec
duration to left S1 root was achieved using
2 bended catheters 10 mm exposed length. Routine closure of the
wound.
Smooth postoperative recovery.
The power of left foot became normal. The patient
is sciatica free. He was sent to the ward.

Comments
The patient still having an estimated
postoperative recurrence around 7%.
This is the 38th case using the BPRF mode
with MultiGen. This procedure regained routine acceptance it
became
a usual part of the spine surgery.
