65 YEARS EXTRUDED DISK C4-5 WITH SPINAL CORD COMPRESSION, CENTRAL MORE TO
THE LEFT.
Anamnesis
The patient came to the clinic 13-June-2004
complaining of nasal bleeding for 30 years with
excision of the hydatic cyst of the liver and
lung 5 years ago, LBP for 10 years, left
sciatica for 3 years, right sciatica for 2
months, neck and left shoulder pain. Her main
complain was at that time the left sciatica. MRI
lumbar spine performed 13-June-2004 showed
dehydrated all lumbar diski. On examination at
that time, she had pain when turning the head to
the right, down and bending the head to the left
shoulder. There was weak grip, extension and
triceps left upper limb. SLRS was 70 degrees
with pain left side. There was aseptic arthritis
right knee with swelling. MRI cervical spine
requested , but the patient disappeared. The
patient then came 01-September-2016 complaining
of numbness both hands with neck pain for 1
weeks.
On examination: the patient has pain when
looking up, down and turning the head to the
left shoulder. There is
weak flexion
and extension of both hands 3/5. Both triceps
-4/5. There is no myelopathic syndrome.
The patient was sent for investigations and MRI
cervical spine performed 18-September-2016
showing central extruded disc more to the left
with spinal cord compression with malacia of the
spinal cord. Dynamic studies showed anterior
fusion of the the C5-6 with calcification of the
anterior longitudinal ligament. The patient was
sent for cardio evaluation.
Diskectomy C4-5 was performed and
the extruded disk was removed until the dura was
seen at the posterior projection of C4-5 level. Using MultiGen
failed due to narrow angles of the field. 1 cervical cage
4x15x13
was inserted to C4-5
level and ACP 18 mm length with 4 fixed
screws 4x18 mm were used to fused C4-5. All
stages of surgery were performed with C-arm
guidance. Routine closure of the
wound.
Smooth postoperative recovery. The power of
both upper limbs normalized and she was sent to the ward.
MultiGen
Comments
The recurrence rate in cervical disk
surgery is zero.
This is the 83d case using the BPRF mode
with MultiGen. This procedure regained routine acceptance.
It
became
a usual part of the spine surgery.

In comparison to the lower spine the
ILLICO system failed to be used in anterior cervical
approach.
The catheters of MultiGen were difficult
to use in this case.
