The patient was operated for PLD L4-5 18 months ago and for
recurrence of PLD L4-5 left side 06-October-2005 ago by me. He was fine until 10 days
ago, he started to suffer from the same left sciatica and MRI
performed confirmed the presence of recurrence at L4-5 left side at
the site of the last surgery.
The patient underwent exploration of
the left upper corner of the previous surgical field and the
extrusion was removed. The disc space was empty and the medial part
of the left L4-5 facet was fractured and flail, for what, it was
removed.
Comments:
1. The cause of the recurrence is not clear, but the presence of
the fractured part of the facet joint, could explain, that the
patient suffered extreme stress to the area, causing the recurrence
to take place.
