03-MARCH-2011 MUHAMED ALI AL-TURK 48 YEARS
SECOND RECURRENCE OF SCHWANNOMA OF THE LEFT SCIATIC NERVE LOWER THIRD OF THE
FEMUR.
Anamnesis
The patient
was was operated by me for schwannoma of the
left sciatic nerve at the lower third of the
femur 1997 and
30-June-2002 for the first recurrence. He
was doing well and neurologically free. The
patient came to the clinic 21-February-2011
confirming that there is enlargement of
the area the last 3 months with soft tissue
compartment without proper palpation of the
boundaries.
MRI of the area
done 24-February-2011 showing huge recurrence of
the schwannoma.
On
examination: The patient was
neurologically free.
The old scar was refreshed and further 2 cm
extended upward. Total resection of the four
separate tumors was achieved. One was
originating from the tibial division of the left
sciatic nerve, the other was originating from
the common peroneal nerve and 2 smaller ones
originating from the sensory branches. The tumor
total weight was around 400 mg. Inspection for
other tumors was negative. All the major nerves
were kept intact.
Routine
closure of the wound. Smooth postoperative
recovery without any neurologic deficit.
Anamnesis
The patient 02-April-2011 with the wound
still oozing and daily dressing was recommended.
He was covered with antibiotics.
The final
histologic result was the same as previously:
benign schwannomas. ( Dr. Salah Al-Jitawi
FRCPath, FIAC).
The
patient then came 28-April-2011 with regression
of the infection.

Comments
The tumor was sent for
histological verification so as to rule out
malignant transformation of the tumor.
The four separate tumors originating from
several nerves explain why recurrence took place 2 times.
