PROGRESSING NORMAL PRESSURE HYDROCEPHALUS WITH MALPOSITIONED VENTRICULAR END OF
THE SHUNT.
Anamnesis
The patient was operated by me
07-December-2014 and VPS inserted from the
right side. The patient showed improvement of
the power of four limbs, but the gait became
worse and has elements of left sided loss of
coordination. MRI of the brain was repeated the
next day, because he was operated late in the
night. The hydrocephalus still the same. The
shunt is not traceable. CT-scan done showing
that the ventricular end is directed
extraventricular to the area of striatum and
globus pallidus.
The cranial part of the shunt was exposed and
the ventricular part of the shunt was released
and withdrawn out. Using stylette, the
ventricular tube was redirected to the lateral
ventricle after what the stylette was removed.
The device was connected together.
Smooth postoperative recovery.
Comments
In the fist surgery, the CSF came out at
first insertion guided with stylette. When it was
re-inserted the second time it was without stylette and it
took another track.
Do not trust that the shunt will go
through the first track when you are reinserting it. All the
time use the stylette.
