EXTRUDED DISC L5-S1 WIDE BASED WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic
23-May-2016 complaining of left sciatica for 45
days. MRI lumbar spine performed 03-May-2016
showing huge disk L5-S1 with left downward
migration. Dynamic studies ruling out gross
overmobility.
On examination: the patient is not limping with
mild scoliotic
stance. SLRS was
70 degrees left side with pain. There is weak
dorsiflexion left foot -4/5 and planterflexion
same foot -4/5.
Using the C-arm,
bilateral foraminotomy both S1 roots was
achieved.
The bony structures were inspected and overmobility
was negative. The extruded disk was removed from
left side and left sided cleaning of L5-S1 was
achieved. Using MultiGen, stimulation of left nerve was achieved with
0.9 V. A
bipolar pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 msec
duration to left S1 root was achieved using 2 bended catheters 10 mm exposed length. Further
motor stimulation done to the same root and the
same response was noted. Routine closure of the
wound.
Smooth postoperative recovery. The patient
is sciatica free. The power of left foot
normalized and he was sent to the ward.

Comments
The patient still have an estimated
postoperative recurrence around 7%, because the disc space
is still not shallow.
This is the 57th case using the BPRF mode
with MultiGen. This procedure regained routine acceptance it
became
a usual part of the spine surgery.
