23-JANUARY-2016 SALEM MUHAMED SALEM 63 YEARS HUGE
RECURRENT DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 12-January-2016
complaining of LBP with right sciatica for 8
months down to the right ankle. MRI lumbar spine done
28-December-2015 showing huge recurrent extruded disc
L4-5 with right downward migration. The patient
was operated at KHMC 2009 for PLD L4-5 for left
sciatica, complicated with right sciatica for
what he was operated 5 days later for recurrence
of L4-5 right side. The patient is a known
hypertensive for 15 years under treatment and
taking Baby aspirin.
On examination: the patient is limping with scoliotic
stance. SLRS was 60 degrees with pain in the
right. There is weak dorsiflexion
right foot -4/5.
Right L4-5 flavotomy with
right foraminotomy L5 root. The extruded disc was
removed lateral to the axilla and right sided
intradiscal cleaning of L4-5 was achieved. Using MultiGen, a
bipolar pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 msec
duration to the right L5 root was achieved using
2 catheters 5 mm exposed length. Routine closure of the
wound.
Smooth postoperative recovery.
The power of right foot became normal. The patient
is still complaining of sciatica. He was
sent to the ward for observation.

Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
still not completely shallow.
The patient still complaining of left
sciatica.
This is the 28th case using the BPRF mode
with MultiGen. This procedure regained routine acceptance it will be
a usual part of the spine surgery.
